Rape and coerced sex with young girls and teenagers is linked to a host of problems later in life, according to a new study in Swaziland by Unicef and the Centers for Disease Control and Prevention.

The study, based on interviews with 1,244 girls and women in the small kingdom on South Africa’s border with Mozambique, was published last week in The Lancet.

Swaziland is one of the countries hardest hit by AIDS: 26 percent of the adult population is infected. The myth, persistent in Africa, that a man can be “cleansed” of AIDS by sex with a virgin puts girls at particular risk, said the authors of an editorial accompanying the study.

The victims had higher lifetime risks of venereal diseases, pregnancy complications and depression. About 5 percent of those interviewed said they had been forced into intercourse before they were 18, and a third said men had tried to rape them or otherwise pressed nonconsensual sex on them. The most common perpetrators were not strangers but men or boys from the neighborhood; 17 percent were male relatives or family friends, almost 3 percent were teachers or principals.

That pattern, that most rapists of girls are known to or related to them, “is common across many cultures,” the authors wrote.

Previous studies have concentrated on sexual abuse in school, and newspaper stories about girls pressured by their teachers to have sex are common in Africa. But the study found that most incidents took place at home, the homes of relatives or friends, or in public areas, including the grasslands outside rural villages.

More than 99 percent of the girls said they were not drunk or using drugs when it happened; in less than a third of the time did they suspect that their attacker was drunk.
